Pathways is one of the largest providers of community based behavioral and social services in America. In 18 states and the District of Columbia, more than 5,000 dedicated Pathways employees create healthier communities by addressing social determinants of health and providing home and community-based services.

Pathways was founded in 1997 and provides direct services and case management for clients of all ages, including Youth & Family Services, Adult Services, and Prevention Services.
